I may be wrong on this, but I'm going to take a wild massive guess on the plot.

What if The Phantom Pain took place in Big Boss's head while he's in a coma?
Maybe that would explain the scenes that include the whale, the unicorn, and the hospital. Again, this is just a big guess that came to me after watching the video.

Or, what if the scenes I mentioned are pretty much hallucinations as snake is escaping?

"Two phantoms were Born." I'm going to guess it's Big Boss and Miller, and that Miller is XOF while Snake is FOX.

Yeah that one. If Joakim Mogren wasn't a really person but rather a character created by the Fox engine, then that's pretty badass.
